Learn Who We Are

  • Kellie D. Lindquist, DDS

    Dr. Lindquist has been at Family Dentistry S.C. since 1998. She graduated from St. Olaf College and then the University of Minnesota School of Dentistry in 1998 with High Distinction. 

    Dr. Lindquist is a member of the American Dental Association and the Wisconsin Dental Association.  In February 2016 Dr. Lindquist completed the certification for BOTOX.

    Dr. Lindquist is married and has 3 children.  

    Her hobbies are traveling, reading, gardening and watching her children's sports and dance.  Dr. Lindquist is a member of the St. Croix Valley and Polk County Master Gardeners Association.


  • Matthew J. Welch, DDS

    After graduating with a degree in Biology from Creighton University in 2009, Dr. Welch received his DDS degree from the University of Minnesota School of Dentistry in 2013.  He is a member of the American Dental Association, the Wisconsin Dental Association, and the Northwest District Dental Society.  

    Dr. Welch has enjoyed providing excellent dental care to the New Richmond and Roberts communities since joining Family Dentistry in 2015.  He is experienced in all aspects of general family dentistry, including cosmetic dentistry, endodontics, Invisalign orthodontics, oral surgery, and pediatric dentistry.  Dedicated to providing gentle, compassionate care tailored to each patient's needs, he strives to make dentistry relaxing and enjoyable, even for the most apprehensive patient.

    In his free time, Dr. Welch enjoys spending time with his wife Kirsten and daughter Lyra.  Otherwise, he likes to get outside running, biking, hiking, camping, playing tennis, and going to soccer games.

  • Douglas G. Stai, DDS

    Dr. Douglas Stai is a graduate of the University of Minnesota-Morris and the University of Minnesota School of Dentistry.  After graduation, he served at the UM School of Dentistry as a part-time faculty member in the crown and bridge department, teaching dental students in the pre-clinic and clinic areas.  He is a member of the Wisconsin Dental Association and the American Dental Association.

    Dr. Stai's dental practice is at Family Dentistry, S.C. in New Richmond and Roberts, WI.  His commitment to current dental techniques and materials has led him to be active in numerous hours of continuing education classes, including cosmetic dentistry and dental implants.  He enjoys the day-to-day contact with patients while providing professional and compassionate care.

    Dr. Stai is a long-term New Richmond resident with his wife, Lori, and enjoys spending time with her, their three daughters and two sons-in-law.  Dr. Stai's interests include photography, reading, fishing, kayaking, canoeing, biking, and downhill skiing.

  • Evan J. Koubsky, DDS

    Dr. Evan Koubsky, originally from St. Cloud, Minnesota, received his DDS degree from the University of Minnesota School Of Dentistry. After living in the Twin Cities and briefly in Eau Claire, he and his wife Luanne (Stai) Koubsky, a New Richmond native, could not be happier to call Wisconsin home.Dr. Koubsky chose to practice at Family Dentistry because of a shared belief in providing state of the art, relationship-based, comprehensive care tailored to fit the lives of his patients. 

    He is dedicated to providing gentle, preventative dentistry with a goal of increasing the longevity of his patients' teeth and overall oral health.

    Prior to dental school, Dr. Koubsky attended North Park University in Chicago, IL where he received a Bachelor of Science in biology with a minor in chemistry. He is a member of the American Dental Association and Wisconsin Dental Association. 

    Outside of dentistry, Dr. Koubsky enjoys spending his time outdoors whenever possible--whether that be camping, fishing, golfing, running, cheering on his favorite sports teams (Go Wild!) and spending time with his family and friends.
